### Extraction & Coverage Rules:

Then, across the full finished set, shuffle option order per question as needed so the correct answer lands in each of positions A/B/C/D roughly 25% of the time, with no repeating cycle. Do this as a final pass after all questions are drafted and verified — never mid-draft, and never in a way that changes a question's content or forces a weaker distractor into place.

### MANDATORY VERIFICATION PASS (do this before the position pass):
After drafting all questions internally, re-check EACH one against the source images:
